The waitresses that were fired and the camera crew have both participated in "ask me anything" forums on reddit, and the yelp reviews going back three years all complain about this type of behavior. As hard as it is to believe, none of this was set up or staged. According to the camera crew, there was a lot more footage that was taken that was not shown including the police showing up as a result of the incident at the beginning of the show.

Thanks for that.

Seems some of it was "staged" to some degree, but only to the extent that people came to eat there because the show was filming. They probably knew/know what the show was about and purposely sent their food back. Whether it was bad enough to send back or not, having "extra" people sending food back probably made matters somewhat worse at least. Still no excuse for her or his behavior though.
