There's nothing wrong with hitting the high spots when sharpening to even out stone wear. But yes, you do need to flatten anyhow.

Yes you need to lap your stone to keep it flat. I sometimes use those high areas to sharpen the shorter knives before I flatten it just because that material is going to be ground away anyway and you'd get no use out of it. That won't flatten the stone good enough. It will just leave dishes in other parts of the stone. After you have done that, flatten the stone completely. That's just a suggestion to get the most use out of the stone.
